Office Policies
All relationships have a better chance of success when all parties are aware of their responsibilities. This is true of the physician-patient relationship. Therefore, we have developed the following policies and procedures to explain our operational and financial polices so we can better work as partners in your care. If you have any questions about our office policies, feel free to ask us for an explanation.
Appointment Policy
Office Hours:
Our Office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. We close for lunch from 12:00 noon until 1:00 p.m. each day.
It is necessary that we see patients by appointment and cannot take walk-ins. Most of our patients are referred to us by their primary care or other personal physician. New patient appointments are generally made by the office of the referring physician. Call our main number 770.534.7885 to make or change an appointment.
We make every effort to see each patient in a timely manner, however emergencies occur that occasionally cause delays in our schedule. Patients who arrive with their paperwork completed or who arrive at least 15 minutes early to complete their paperwork in the office will receive first priority to be seen. Patients who do not have their paperwork completed by their scheduled appointment time will be given the opportunity to reschedule their appointment.
If we have a correct telephone number, we will attempt to remind each patient of his appointment by telephone as a courtesy. It is still the patient's responsibility to remember their appointment. If you need to cancel or reschedule your appointment, please notify us more than 48 hours in advance if possible so we may contact those persons on our waiting list for an earlier appointment.
Policy for Missed Appointments Without Notice:
Failure to give a 48 hour notice will result in a $25.00 charge assessed to your account of which insurance will not pay.
For you to achieve the optimum results from your treatment it is important to come to your appointments for rechecks as required by your treatment plan. Patients who repeatedly fail to keep their appointments, may be discharged from the practice.
Insurance & Financial Policy
Although we accept assignment from the health plans listed, payment of required deductibles, co-payments, co-insurance and the costs of non-covered services are still your responsibility. We will be glad to file a claim for your primary insurance, however we will request payment of your portion of our fees at the time of service. Office co-pays will be requested prior to visit. We cannot file for the medical benefits portion of your automobile insurance policy therefore you will be required to pay the cost of the visit in advance and you then will have to file your automobile insurance for reimbursement. We are unable to establish payment plans. However, for your convenience we accept all major credit cards, (Mastercard, Visa, American Express, Discover and debit cards along with cash and checks).
If your account is turned over to a collection agency for nonpayment you will be charged an additional 40% for the cost of collecting the overdue account by the collection agency. In addition, you may be discharged from the practice.
Pain Medication and Refill Policy
As a patient of Gainesville Neurology Group, LLC, I:
- Will keep all appointments as recommended.
- Agree to allow 48 hours for prescription refills.
- Understand that prescription refills requested after 4:00 p.m. will not be seen and processed until the next working day.
- Understand that a follow-up visit may be required from my physician in order to obtain a refill.
- Agree to take all medication exactly as instructed and am not allowed to change the dosage amounts or scheduled time to take th e medication without first speaking to my physician.
- Understand that prescription refills will not be phoned in after-hours or on weekends as the on-call physicians do not have access to patient records at those times.
- Understand that GNG will not refill prescriptions that have been lost, stolen or otherwise misplaced.
- Will not combine any narcotic medication with the consumption of alcohol.
- Understand that I may be terminated from the practice with 30 days notice for noncompliance in the taking of their medications.
Am aware that I will be terminated from the practice immediately if I:
- Obtain narcotics from any other physician while under our care for a condition for which we are the main treating physician.
- Give, trade, or sell medication to others.
- Alter or forge a prescription (this is a felony and will be reported).
- Understand that the physician may refuse to prescribe medication if I, the patient, do not agree to these terms.
